Tecumseh High School and Middle School will no longer accept sports physicals in paper form, only electronic copies uploaded to your FinalForms Account. Any student wishing to participate in school athletics must have a current physical uploaded to their account in order to be eligible for participation prior to the first day of practice. A current-year physical is one given on or after April 15th of the previous school year. Follow the instructions below on registering your account if you have not already done so and/or uploading your physical. 

How do I upload my student’s sports physical?

1. Login and click UPDATE FORMS.

2. Click on “MHSAA PPE Physical”, located on the left hand side at the bottom of the form list, and follow the instructions to upload the physical digitally. 

3. Take a picture of your completed physical

• A current-year physical is one given on or after APRIL 15 of the previous school year.

• Physicals must be signed by a MD, DO, PA, or NP. 

4. Click the black box “Upload Physical” and select the file you saved as the picture of the physical.

5. Once the file is uploaded, click the blue box “Submit Form” 

6. Submitted physicals will be reviewed and accepted by a school official. Once accepted and all forms have been signed in yours and your student’s Final Forms accounts, your student’s account will be flagged as eligible to participate in school athletics.

•  All physicals are given an expiration date of June 30th within the Final Forms System.
